What you give us on this website

Booking an appointment asks for your first name, last name, email, and phone number, plus the service, practitioner, and time you choose. A short message is optional. The contact form asks for your name, email, and message, and a phone number if you add one. We use those details to confirm the visit, answer you, and, when email sending is set up, send one reminder about 24 hours before a confirmed appointment.

Patient accounts

If you create a patient account, you use the email from your visit and choose your own password. The clinic does not set or see that password. After the account is linked to your record at this clinic, you can see your own appointments and messages the clinic has already saved for you. Treatment notes are not shown in the patient account. The same email at another clinic is a separate record.

What the public forms do not ask for

The booking and contact forms do not ask for a health history, a diagnosis, or an insurance number. Please do not put those details in the optional message. Health information collected during your care at the clinic is kept as part of your clinical record and is handled under the clinic’s professional duties, including Ontario’s health privacy law. This draft website notice is not that full clinical privacy statement.

Who can see it

Clinic staff who need the information to book, provide care, or answer you can see it. The website is operated on ChirOS. Service providers that host the site, store appointment records, and send email handle that information only so those services can run. We do not sell personal information.

How long it is kept

Appointment and contact details are kept so the clinic can provide the visit and answer questions about it. Clinical records are kept for the period required by the clinic’s professional obligations. A finished policy will state those periods exactly.

Seeing or correcting your information

You can ask to see or correct the contact and appointment details we hold by emailing or calling the clinic. If you have a patient account, you can also review your own upcoming and earlier visits there.
